Release and Terms & Conditions for Partnership Intake Form

Effective Date: February 7, 2025

1. Authorization and Consent
By completing and submitting the Partnership Intake Form (“Form”), the individual or organization (“Partner”) expressly authorizes Vet Info, Inc. (“Vet Info”) to publish, display, and use the content, images, logos, and information provided in the Form (collectively, “Submitted Materials”) on its website and other informational platforms. This authorization extends to purposes that directly support the needs of the veteran community and promote related informational activities.

2. Scope of Use
Vet Info may use the Submitted Materials in the following ways:

• Displaying Partner information on its website and affiliated platforms.
• Utilizing logos and images for promotional or informational content.
• Sharing content that supports veterans’ services, programs, and partnerships.

3. Ownership and Rights
The Partner retains ownership of its respective intellectual property, including logos and images. However, by submitting the Form, the Partner grants Vet Info a non-exclusive, royalty-free, worldwide license to use, reproduce, modify, and display the Submitted Materials for the purposes stated herein.

4. Modification and Removal of Content
If the Partner wishes to withdraw consent or have their information removed from Vet Info’s platforms, a written request must be submitted to info@vetinfo.org. Vet Info will make reasonable efforts to remove the requested materials within thirty (30) business days of receiving the written notice.

Vet Info also reserves the right to remove the Partner and any related information from its website and affiliated platforms at any time, without cause or reason, and is not required to provide an explanation for such removal.

5. Representations and Warranties
By submitting the Form, the Partner represents and warrants that:

• They have the authority to provide the Submitted Materials and grant the rights described herein.
• The Submitted Materials do not infringe upon the intellectual property rights of any third party.
• The information provided is accurate and truthful to the best of their knowledge.

6. Limitation of Liability
Vet Info will not be held liable for any indirect, incidental, or consequential damages arising from the use or display of the Submitted Materials, except where prohibited by law.

7. Amendments
Vet Info reserves the right to amend these Terms & Conditions at any time. Any updates will be posted on the Vet Info website, and continued participation constitutes acceptance of the updated terms.

8. Governing Law
These Terms & Conditions shall be governed by and construed in accordance with the laws of the State of [Insert State], without regard to its conflict of law principles.
